Immunization Information

New York State Law Section 2164 which requires immunizations for school attendance has undergone several changes in the past few years.  Updated proof of immunization must be sent to the school nurse within 14 days of the start of the school year or your child will be excluded.

Proof of immunization must be
any 1 of the 3 items listed below:

  • An immunization certificate signed by your health care provider
  • Immunization Registry Report (NYSSIS) from your health care provider or County Health  Department
  • A blood test (titer) lab report that proves your child is immune to measles, mumps, rubella, varicella, hepatitis B and polio

For Varicella (chickenpox), a note from your health care provider (MD, NP, PA) which says your child had the disease is also acceptable.

Please review the link below to review NYS immunization chart and footnotes.  In addition, check with your child's health care provider to ensure they will be up to date when school resumes in September.
If you have questions or concerns, please consult with your school nurse.
