Immunization Information

New York State Law Section 2164 which requires immunizations for school attendance has undergone several changes in the past few years. Updated proof of immunization must be sent to the school nurse before the start of school.

Proof of immunization must be any 1 of the 3 items listed below:

    • An immunization certificate signed and stamped  by your health care provider
    • Immunization Registry Report (NYSSIS) from your health care provider or County Health Department
    • A blood test (titer) lab report that proves your child is immune to measles, mumps, rubella, varicella & hepatitis b.
Please review the link below to review NYS immunization chart and footnotes. In addition, check with your child’s health care provider to ensure they will be up to date when school resumes in September. If you have questions or concerns, please consult with your school nurse.
